If the condition is a result of an auto accident, written authorization is needed prior to the appointment. Most auto companies will not give authorization however will supply in writing a notice of an open claim with current billing information.
Copay amounts are due at the appointment unless other arrangements have been made with the office manager in advance. The office accepts payment in the form of cash, check, VISA or MasterCard. If there is no insurance, please make arrangements with the office manager. A financial agreement can be arranged for payment.
